Armandee Drew Coaching — Consent Form

I understand and agree to the following:

1. Nature of Services

I understand that Armandee Drew will use a variety of approaches to best serve me. One of these techniques is a mind-body approach intended to neutralize distressing emotions and disempowering beliefs, and to align me with my goals. Any information provided by Armandee Drew is educational in nature and is to be used at my discretion.

2. Possible Aftereffects of Sessions

I understand that I may experience emotional shifts or what are sometimes called "parts waking up" between sessions. These experiences are more likely if I have a history of chronic or acute trauma, or if I have experienced addiction. If such experiences occur, I will use my personal regulation strategies, and if those are not helping, I agree to contact my coach.

3. Coaching Is Not Medical or Psychological Treatment

I understand that coaching with Armandee Drew is not a substitute for medical or psychological treatment or diagnosis, and that Armandee Drew does not diagnose, treat, or prescribe. Any mental, emotional, or medical concerns should be addressed with a qualified physician or licensed psychotherapist. I agree to disclose to Armandee Drew any mental, emotional, or medical concerns relevant to my participation in coaching.

4. Voluntary Participation and Consent to Touch

I understand that participation in coaching is voluntary, and that I may choose to end my participation at any time. I understand that some techniques, during in-person sessions, involve Armandee Drew holding my right hand and wrist. If such a technique is appropriate, she will ask for and obtain my consent before any physical contact occurs.

5. Confidentiality

I understand that information exchanged with Armandee Drew is confidential and will not be released without my prior written consent, except as required by law.

6. Assumption of Responsibility; Release

I understand that by providing this informed consent, I am assuming full responsibility for my participation in coaching sessions, for what I say, and for my choices. I agree to hold harmless Armandee Drew and the facility or location where sessions are provided, to the fullest extent permitted by law.

7. Dispute Resolution

Should any claim arise out of these coaching services, I agree to first seek resolution through mediation, and if unsuccessful, through binding arbitration, governed by the laws of the State of Georgia.

8. Payment, Cancellation, and Email

Payment is due at the time of service. Because appointment time has been reserved exclusively for me, I understand that at least 24 hours' notice is expected for cancellations, and that missed appointments will be charged 50% of the session fee. Insurance is not accepted.

I agree to receive emails from Armandee Drew.

9. Questions and Concerns

I understand that if I have any questions or concerns about coaching services, I will raise them promptly with Armandee Drew.

10. Authorization

I hereby authorize Armandee Drew to provide me with coaching session(s). I confirm that I have read, understood, and agreed to the terms above.
